Manajemen Anestesi pada Seksio Sesarea dengan Serangan Asma

Abstract

Seksio sesarea adalah sebuah prosedur dimana bayi dilahirkan melalui sebuah insisi pada dinding abdomen dan uterus ibu hamil. Selama persiapan dan pelaksanaan seksio sesarea, pada ibu hamil dengan asma, dapat terjadi serangan asma, yaitu hiperresponsif jalan nafas (respon penyempitan dan edema jalan napas yang berlebihan terhadap pemicu, seperti alergen dan olahraga), dengan gejala mengi, dispnea (sesak napas), dan batuk. Serangan asma ini dapat diakibatkan pelepasan epitel, fibrosis subepitel, peningkatan jumlah dan volume sel mukosa di epitel, hiperplasia otot polos jalan napas, dan hipertrofi, serta peningkatan vaskularisasi dinding jalan napas. Manajemen serangan asma pada seksio sesarea dipengaruhi oleh teknik anestesi yang digunakan. Manajemen serangan asma pada seksio sesarea dengan anestesi regional, dapat menggunakan lidokain 1-2 mg/kgBB IV. Serangan asma pada seksio sesarea dengan anestesi umum dapat dicegah dengan kortikosteroid inhalasi seperti beclomethason 400 μg per hari. Perawatan harus diambil untuk mencegah aspirasi selama intubasi. Jika mungkin, pasien harus ditempatkan dengan kepala tempat tidur ditinggikan untuk mencegah pneumonia terkait aspirasi dan eksaserbasi asma
